LEASING PROCEDURES
Complete and return the enclosed application. If your tractor is older than a 1995 model, include a recent photo of the tractor.
Once your application is approved, we will schedule a date for you to come to Griffith, Indiana with your equipment.
You must Fax the following to us before orientation:
1. Title or Application for Title in the owner/operators name
2. Form 2290 proof that the current year
Federal Highway use Tax has been paid
3. Valid Annual D.O.T. Inspection for equipment
4. Scale Ticket for equipment
5. Long Form Physical and physical card ( Less Than 1 Year Old) 6. CDL with required endorsements
7. Social Security Card or Birth certificate
8. Certificate of Insurance for insurance’s not
purchased through Powersource
9. Sign On Money
Be prepared to spend 1-1/2 days for orientation. Powersource will provide one nights lodging. You will be dispatched on the afternoon of the second day from Griffith, IN.

INSURANCE
BOBTAIL INSURANCE: REQUIRED
You may purchase through Powersource group policy at the cost of $35.00 per month. Initial outlay (at time of sign on) of $35.00 is the first month premium.
OCCUPATIONAL/ACCIDENT: REQUIRED
You may purchase through Powersource group policy at the cost of $142.00 per month. Initial outlay (at time of sign on) of $142.00 is the first month premium.
CARGO INSURANCE: REQUIRED
The main cargo policy is provided by Powersource Transportation. This cargo deductible policy covers the first $5000 of cargo insurance with $500 deductible for the owner/operator. This coverage is required to be carried through Powersource at the cost of $45.00 per month. Initial outlay (at time of sign on) of $90.00 includes one month deposit and first month premium.
PHYSICAL DAMAGE: AVAILABLE
Coverage is available at group rates. Contact Powersource for a quote. Initial outlay requires the first month premium.
LIABILITY INSURANCE
Coverage provided under Powersource Transportation Policy.
ESCROW
A $2,500.00 escrow is required. You are required to put $250.00 down at sign on. Seventy-five dollars will be deducted from the settlement until the full amount is in escrow. In the event of termination of contract, this money will be returned forty-five days after receipt of all property belonging to Powersource Transportation, provided all financial obligations to Powersource are met.
